micropenis: Encyclopedia II - History of intersex surgery - Complications arise both in the theories and in the patients

Throughout the 1980s pediatric surgery textbooks recommended female assignment and feminizing reconstructive surgery for XY infants with a severely inadequate phallus. Nevertheless, in the 1980s several factors began to induce a decline in the frequency of certain types of genital surgery. Pediatric endocrinologists had realized that some boys with micropenis had deficiency of growth hormone which could be improved with hormones rather than surgery, and over the next decade a couple of reports suggested adult outcome as males was not as bad ...

micropenis: Encyclopedia II - History of intersex surgery - Controversy

The goals of surgery, outlined at the beginning of this article have not changed, but some types of surgery have been discontinued. The primary ongoing controversy surrounds timing of feminizing surgery for virilized girls with CAH. Alhough surgical sex reassignments (to female) of XY males with unambiguous micropenis or penile injury had been dwindling in the 1990s, few if any have been done since. Accounts of spontaneous self-reassignment since then have also reduced the reassignment to female of several other forms of XY ambiguity ...

micropenis: Encyclopedia II - Prenatal hormones and sexual orientation - Links between adult sexual orientation and gender behaviour as children

Physiological factors present at birth may produce behaviours in children that are interpreted as gender difference. Such gendered behaviour may be an indicator of adult sexual orientation — a 1987 longditudinal study suggested that 75% of sissy boys grow up to be gay or bisexual adults (Green 1987). Cornell psychologist Daryl Bem has proposed a theory called “the exotic becomes erotic”. In it he suggests children with Childhood Gender Nonconformity (CGN), a biologically-caused attribute, are often ostracized by their own sex fo ...

micropenis: Encyclopedia II - Kallmann syndrome - Epidemiology

Kallmann syndrome occurs at a rate of 1 in 10,000 male births and 1 in 50,000 female births. It may be inherited as an X-linked condition, an autosomal dominant condition or as an autosomal recessive condition. Statistics are sparse, but it seems that autosomal dominant is the most common form of heredity. Even though mutations in the KAL gene on the X chromosome can cause Kallmann syndrome, only 11-14% of patients with Kallmann syndrome have detectable KAL mutations. Mutations in autosomal gene FGFR1 have been found in some ca ...

micropenis: Encyclopedia II - Kallmann syndrome - Treatment

Treatment is directed at restoring the deficient hormones -- known as hormone replacement therapy (HRT). Males are administered human chorionic gonadotropin (hCG) or testosterone. Females are treated with oestrogen and progestins. To induce fertility in males or females, GnRH (aka LHRH) is administered by an infusion pump, or hCG/hMG/FSH/LH combinations are administered through regular injections. Fertility is only maintained whilst actually being treated with these hormones. Once fertility treatment stops it is necessary to revert to the normal HRT of testosteron ...

micropenis: Encyclopedia II - Cryptorchidism - Causes of cryptorchidism

In most full-term infant boys with cryptorchidism but no other genital abnormalities, a cause cannot be found, making this a common, sporadic, unexplained birth defect. Although severely premature infants can be born before descent of testes, there is a strong association of cryptorchidism with low birthweight due to either prematurity or intrauterine growth retardation. In these infants there is usually no evidence of ho ...

micropenis: Encyclopedia II - Cryptorchidism - Diagnostic evaluation

The most common diagnostic dilemma in otherwise normal boys is distinguishing a retractile testis from a testis that will not/cannot descend spontaneously into the scrotum. Retractile testes are more common than truly undescended testes and do not need to be operated on. In normal males, as the cremaster muscle relaxes or contracts, the testis moves lower or higher ("retracts") in the scrotum. This cremasteric reflex is much more active in infant boys than older men. A retractile testis high in the scrotum can be difficult to distinguish fro ...

Physiological factors present at birth may produce behaviours in children that are interpreted as gender difference. Such gendered behaviour may be an indicator of adult sexual orientation — a 1987 longditudinal study suggested that 75% of boys who exhibit cross-gender behaviour grow up to be gay or bisexual adults (Green 1987). Cornell psychologist Daryl Bem has proposed a theory called “the exotic becomes erotic”. In it he suggests children with Childhood Gender Nonconformity (CGN), a biologically-caused attribute, are often o ...

micropenis: Encyclopedia II - History of intersex surgery - Outcomes and statistics

Regardless of the "representativeness" of the advocacy groups, the doctors began to listen. Within a short time re-evaluation of intersex management was a major topic in the journals and the meetings of the pediatric endocrinologists and urologists. There was no international "registry" of genital reconstructive surgery. Most cases of this type of surgery were being carried out in a relatively small number of centers. Several of the larger institutions embarked on individual and collaborative retrospective outcome studies, attempting to cont ...

micropenis: Encyclopedia II - Sex assignment - Assignment in intersex conditions

Intersex is a broadly defined term that usually denotes the presence of discordance of the biological aspects of sex: at least some aspect of the genitalia, internal organs, gonadal tissue, or chromosomes is more typical of the other sex, or incompletely differentiated. When the external genitalia appear to be "in between", they are described as ambiguous. In approximately 1 in 5,000 infants there is enough variation in the appearance of the external genitalia to give rise to hesitation about appropriate assignment by the physician in ...

micropenis: Encyclopedia II - Growth hormone deficiency - Causes of GH deficiency

There are many causes of GH deficiency. Some examples include: mutations of specific genes (e.g., GHRHR, GH1) congenital malformations involving the pituitary (e.g., septo-optic dysplasia, posterior pituitary ectopia) damage to the pituitary from incracranial disease (e.g., hydrocephalus), intracranial tumors in or near the sella turcica, especially craniopharyngioma, damage to the pituitary from radiation therapy to the head for leukemia or brain tumors, surgery in the area of the pi ...

micropenis: Encyclopedia II - Cryptorchidism - Inheritance and recurrence risk of cryptorchidism

A small percentage of cases of isolated cryptorchidism are familial. It has been reported that about 4% of fathers and 6-10% of brothers of affected boys have also had cryptorchidism. Few specific genes associated with isolated cryptorchidism have been identified. In contrast, many of the genes causing some of the intersex conditions associated with androgen or AMH deficiency or insensitivity have been ident ...

micropenis: Encyclopedia II - Growth hormone deficiency - History

Perhaps the most famous person who exemplified the appearance of untreated congenital growth hormone deficiency was Charles Sherwood Stratton (1838-1883), who was exhibited by P.T. Barnum as General Tom Thumb, and married Lavinia Warren. Pictures of the couple appear to show the typical adult features of untreated severe growth hormone deficiency. Despite the severe shortness, limbs and trunk are proportional. Like many other 19th century medical terms which lost precise meaning as they gained wider currency, “midget” as a term for someone with severe proportional shortness acquired pejorative con ...

micropenis: Encyclopedia II - Growth hormone deficiency - Terminology

The term hypopituitarism is often used interchangeably with GH deficiency by endocrinologists but more often denotes GH deficiency plus deficiency of at least one other anterior pituitary hormone. When GH deficiency (usually with other anterior pituitary deficiencies) is associated with posterior pituitary hormone deficiency (usually diabetes insipidus) the condition is termed panhypopituitarism. HGH also refers to human growth hormone but this older abbreviation has begun to develop paradoxical connotations (see fuller discussio ...

Though growth ends with sexual maturity, GH continues to be secreted throughout life. In adults, GH contributes to maintenance of muscle and bone mass and strength. Understanding of the effects and benefits of GH in adults increased in the 1980's and the features of adult GH deficiency have not only been published, but can be said to have gained some notoriety. Reported effects of severe GH deficiency in adults have included: reduced muscle mass and strength reduced bone mass and strength reduced physica ...

micropenis: Encyclopedia II - Intersex surgery - Masculinizing surgical procedures

Orchiopexy and hypospadias repair are the most common types of genital corrective surgery performed in infant boys, but most of these boys have no other abnormalities and are not considered to have an intersex condition. Undervirilized boys typically have at least one of these two procedures performed in addition to others described below. Types of undervirization and malformation for which some type of masculinizing surgery has been performed most often in the last 50 years are: the configuration of ambiguous genitalia r ...

micropenis: Encyclopedia II - Kallmann syndrome - Pathophysiology

Under normal conditions, GnRH travels to the pituitary gland via the tuberoinfundibular pathway, where it triggers production of gonadotropins (LH and FSH). When GnRH is low, the pituitary does not create the normal amount of gonadotropins. The gonadotropins in turn affect the production of hormones in the gonads, so when they are low, these hormones will be low as well. In Kallmann syndrome, the GnRH neurons do not migrate properly from the olfactory placode to the hypothalamus during development. The olfactory bulbs also fail to for ...

micropenis: Encyclopedia II - History of intersex surgery - Triumph of infant surgery and nurture over nature

By the 1960s, the young specialties of pediatric surgery and pediatric urology at children's hospitals were universally admired for bringing infant birth defect surgery to new levels of success and safety. These specialized surgeons began to repair wider varieties of birth defects at younger ages with better results. Earlier correction reduced the social "differentness" of a child with a cleft lip, or club foot, or skull malformation, or could save the life of an infant with spina bifida. Genital corrective surgeries in infancy were j ...

micropenis: Encyclopedia II - Penis - Linguistics

Penis - Etymology. The word is derived from the Latin word for tail, also used to describe the organ, "penis". The Latin word "phallus" (from the Greek "phallos") is sometimes used to describe the penis, though the word originally was used to describe images, pictoral or carved, of the penis [1]. Some derive the Latin word penis from earlier *pesnis, and the Greek word peos = "penis" from earlier *pesos. Penis - Synonyms. For a far more exhaustive and multi-lingual th ...
