Growth hormone deficiency: Encyclopedia II - Growth hormone deficiency - Diagnosis of growth hormone deficiency

Pediatric endocrinologists are the physicians who specialize in diagnosis and treatment of growth hormone deficiency and growth problems in children. Internist endocrinologists are the physicians with the most expertise in assessment and treatment of adult GH deficiency. Although GH can be readily measured in a blood sample, testing for GH deficiency is constrained by the fact that levels are nearly undetectable for most of the day. This makes simple measurement of GH in a single blood sample useless for detecting ...

Growth hormone deficiency: Encyclopedia - Pituitary gland

The pituitary gland, or hypophysis, is an endocrine gland about the size of a pea that sits in the small, bony cavity (sella turcica) at the base of the brain. Its posterior lobe is connected to a part of the brain called the hypothalamus via the infundibulum (or stalk), giving rise to the tuberoinfundibular pathway. The posterior lobe is thus derived from neural ectoderm while the anterior lobe is derived from oral ectoderm. The anterior pituitary lobe receives releasing hormones from the hypothalamus via a portal vein system. ...

Growth hormone deficiency: Encyclopedia II - Growth hormone treatment - Other GH uses and treatment indications

In the last two decades, GH has also been increasingly used for children and adults who are not severely deficient, either to enhance growth or for other reasons. Growth hormone treatment - GH treatment for other types of shortness. Many conditions besides GH deficiency cause poor growth. GH therapy has been shown to improve short-term growth in many conditions, but long-term height gains are usually poorer than those achieved when GH deficiency is the cause of shortness. Higher ("pharmacologic") doses are typically required to achieve ef ...

Growth hormone deficiency: Encyclopedia II - HGH controversies - Background on HGH and terminology

HGH and hGH refer to "human growth hormone." The reader may be curious as to why only this hormone is still often referred to as "human" when the term could obviously be appended to every hormone in the article Hormone. After three-letter abbreviations of amino acids were found so useful in the 1950s, endocrinologists in the 1950s tended to favor 3 letter abbreviations even for two-word hormones. At that time, most of the hormones available for administration, especially the proteins, were derived from animals. Most were simil ...

Growth hormone deficiency: Encyclopedia II - Growth hormone treatment - Terminology and glossary

Growth hormone (GH) is also called somatotropin (British: somatotrophin). The human form of growth hormone is known as human growth hormone, or hGH (similarly ovine growth hormone is abbreviated oGH). GH can refer either to the natural hormone produced by the pituitary (somatotropin), or biosynthetic GH for therapy (somatropin). HGH is an abbreviation sometimes used for counterfeit or fake "growth hormone" products. See HGH quacke ...

Growth hormone deficiency: Encyclopedia II - Growth hormone treatment - Treatment of GH deficiency in children

Growth hormone deficiency is treated by replacing GH. All GH prescribed in North America, Europe, and most of the rest of the world is a synthetic copy of human GH, manufactured by recombinant DNA technology. As GH is a large protein molecule, it must be injected into subcutaneous tissue or muscle to get it into the blood. Nearly painless insulin syringes make this less trying than is usually anticipated but perceived discomfort is a subjective value. When a person has had a long-standing deficiency of GH, benefits of treatment are of ...

Growth hormone deficiency: Encyclopedia II - Growth hormone treatment - Ethical issues

Hormone treatment seems an unlikely source of social controversy, but for four decades, growth hormone has been second only to estrogens and progestins (diethylstilbestrol, contraception, abortifacients, and post-menopausal replacement) in its ability to engender challenging ethical issues. The principal controversies of the last two decades arise from the intersection of two factors: high cost and the difficulty of defining a boundary between disease and variation of normal. In other words, if GH were inexpensive, it would be no more contro ...

Growth hormone deficiency: Encyclopedia II - HGH controversies - The origin of the HGH industry: Rudman and Klatz

Although endocrinologists and other scientists interested in aging were asking this question as early as the 1970s and there were some uncontrolled and unpublished individual trials, the question became a subject of public interest due to two publications in the 1990s. Rudman D, Feller AG, Nagraj HS et al (1990). "Effect of human growth hormone in men over 60 years old". New Engl J Med 323:1-6. This famous paper from the New England Journal of Medicine reported research by endocrinologist Dan Rudman. It described modest improve ...

Growth hormone deficiency: Encyclopedia II - HGH controversies - Unproven aspects of real GH use in aging adults

The Rudman study attracted much public attention. A number of scientific studies attempted to replicate and address the questions raised by the Rudman study. In general, the studies have shown less impressive benefit and considerably higher rates of side effects when growth hormone has been given to older but non-deficient adults. These reports were less interesting to the news media. For example, Blackman's better controlled trial (Blackman, 2002) of 6 months of growth hormone in 29 women and 36 men confirmed mild improvement in lean ...

Growth hormone deficiency: Encyclopedia II - HGH controversies - Background on hormonal aspects of aging

Most of the changes of the human body after age 30 are unwelcome, and are referred to as aging. For all of recorded history, people have sought ways to slow the changes of aging. In the context of putative anti-aging therapies, the most relevant body changes include diminished muscle mass and strength, bone strength and density, bone size, basal metabolic rate and calorie needs, elasticity of skin and connective tissue, rate of healing, libido and sexual performance, production of body fluids (tears, saliva, vaginal lubrication, joint ...

Growth hormone deficiency: Encyclopedia II - HGH controversies - Adult growth hormone deficiency

The features of adult growth hormone deficiency include poor maintenance of lean body mass, with reduced muscle strength and diminished bone density. The reduced bone density can be severe enough to become osteoporosis. The body composition of GH-deficient adults tends to include a higher percentage of body fat. Cholesterol levels are higher, as is risk of cardiovascular disease. Subtler or more subjective p ...

Growth hormone deficiency: Encyclopedia II - Growth hormone treatment - Risks of GH treatment

Known risks of GH are few and rare. Few reasonable parents or physicians would incur a high risk of harm to a child to add a few inches to height. Most of the complications have been reported in children over 10 years of age or in adults. Though rare, the following harmful side effects have been reported during GH treatment often enough to be assumed noncoincidental. Slipped capital femoral epiphysis (SCFE) causes hip pain due to separation of the head of the femur from the shaft. Incidence in GH-treated children may be about 1 ...
